| HWC Students! Welcome to the Harold Washington College
Employee of the Month Award online voting form. We encourage you to take time out of your busy
academic schedule to help us recognize the good work that HWC faculty and staff members do on a
daily basis, by submitting to us information about a faculty or staff member who has made a
difference for you at HWC.

| To use the form below to vote online, we ask that you submit your name, your
student email address (Format: username@student.ccc.edu), the name of an HWC faculty or staff
member who you would like to nominate, and a description of something that the HWC faculty/staff
member nominee did for you or someone else that impressed you. If you do not have a City Colleges
of Chicago email address, visit the City Colleges of Chicago Student Email website at
http://student.ccc.edu
to obtain your campus student email account. You must have a City Colleges of Chicago student
email account in order to use this online voting form!!!

| After entering the requested information in the above form, please click the 'Submit'
button to submit your results to the HWC Employee of the Month Award Committee, who will pick
a monthly winner based on online and hardcopy submissions. Information about each monthly winner
will be available on the Harold Washington College website home page.
