| Purpose: |

This online form will allow interested students to make a reservation to participate in a
three-hour sanitation seminar for preparing food vendors to operate in compliance with
applicable rules and guidelines for safe outdoor food handling. Classes are offered on Tuesday
afternoon and Thursday evening. All classes are taught by certified instructors.

| All classes are held at City Colleges of Chicago, District Office, 226 West Jackson (corner of Jackson
and Franklin), Room 402.
|
| Cost: |
| $30.00 includes all handouts and certificate. "Payment by money order only!"
Credit card payments and cash can be accepted by the College Cashier when registering in person at
the college. Please make your $30 payment by money order payable to Harold Washington College.

There are 4 ways to register for the "2009 Professional Sanitation Seminar for Summer
Festival Food Vendors using the Class Choice & Registration form document (Adobe
PDF or MS Word).
(Note: In order to complete the registration process, you must arrive 30 minutes before the start
of your first class):
- In Person: at Harold Washington College, 30 E. Lake Street, Room 101, Chicago IL 60601
- By Mail: after downloading the Class Choice & Registration form document and mailing
it to Harold Washington College, Foodservice Sanitation Program Assistant Dean, Attn: Patricia Cuevas, 30 E. Lake
Street, Chicago IL 60601. Please note special instructions about filling out and returning
mail-in registration form.
- By Email: email the Class Choice & Registration form document to Harold Washington

Please note that no walk-in registrations will be accepted on the day of a class. Classes are
conducted in English unless otherwise indicated. You will receive a follow-up call or

- You may only attend class on the chosen date and time.
- Certificates will be issued at the end of the 3-hour seminar.
- The college reserves the right to cancel any class in which enrollment is insufficient.
- No refund will be issued for missed seminar or tardiness.
- Can arrange for class at off site locations for groups of 15 or more students. Call (312)

Discounted parking can be received with HWC validation stamped at the Security Desk
on the first floor at the campus:
- 20 E. RANDOLPH – See attendant in front of the elevators before paying for parking.
- 60 E. LAKE STREET: after 3pm.
- 181 N. DEARBORN: after 4pm.
